The electronic Hurley Score, is based on the classification system developed by Hurley in 1989 to categorize the severity of Hidradenitis Suppurativa (HS), also known as acne inversa, based on clinical appearance. The Hurley Score helps dermatologists to assess the severity of HS and determine appropriate treatment strategies. However, as it is too static, the tool is not suitable to monitor the progression of the disease over time.

The eHurley Score should only be used by healthcare professionals. The score is divided into three distinct stages:

Hurley Stage I: Characterized by the presence of single or multiple abscesses, without sinus tracts or scarring.

Hurley Stage II: In this stage, multiple recurrent abscesses are present, along with the formation of sinus tracts and scarring. The affected area may be more extensive, with interconnected lesions.

Hurley Stage III: This is the most severe stage, with widespread involvement and multiple interconnected sinus tracts and scarring. This stage is associated with significant functional and quality of life impairment.

At present, the Hurley classification remains valuable for categorizing HS into three levels of severity. However, it has certain limitations: The Hurley classification lacks quantity and dynamic components as it is only based on three stages and relies on static disease features such as scars and fistulas. Other tools (also available on DermaValue), such as the Sartorius Score or the IHS4, are more detailed and dynamic compared to the Hurley Score.
Still, the current recommendation is to use the Hurley Score to guide treatment decisions, where a severe stage may require a more intensive treatment strategy.
